Organization Forms/Resources

  • Recognized Student Organization Information Form – Each year, Organization Officers are required to sign this document. This form acts as our agreement with student organizations for University recognition. It provides leaders with a comprehensive list of all policies and regulations in regard to student organizations. This form is to be completed by all RSOs at the start of the school year.
  • Change of Information Form – This form helps ensure U&I has the most current information for student organizations. We ask that this form be submitted anytime there is a major change in organizational leadership or information.  This form is to be completed when there are changes in leadership during the year. Examples include: President/Primary Contact change, Advisor Changes, Website Address change, or Organization Name changes.
  • RSO Advisor Agreement Form – This form is to be filled out and signed by both the Advisor and President/Primary student at the start of each year. Make sure to send the filled out PDF to orgs@truman.edu.
  • Student Organization Recognition Agreement Form – This form is to be filled out by the President/Primary student of the organization at the start of each year. The easiest way to turn this in is via email or if you have Acrobat you can use that to send the PDF. Don’t forget to send itto orgs@truman.edu.
  • Anti-Hazing Non-Discrimination Form – This form is to be read and signed by all members of a chartering organization in order to show the understanding of the Anti-Hazing and Non-Discrimination Policies. The easiest way to turn this in is via email or if you have Acrobat you can use that to send the PDF. Don’t forget to send it to orgs@truman.edu.
  • Student Organization Disposition of Funds Form – This form is to be filled out by chartering organizations only. Don’t forget to send it to orgs@truman.edu.
  • Copy Code Activation Form – This form is for organizations who would like to gain access to the copier located on the lower level of the SUB. Fill out the information to receive your copy code. 

  • Physical Risk Module Quiz – This should be done by all organizations who are inherently physically risky. This quiz should be done in the fall semester (usually) or whenever there is a new risk manager. For the 2020-21 school year, the deadline will be in the spring semester.

  • Financial Risk Module Quiz – This should be done by all organizations who do anything financial. The quiz should be done in the fall semester (usually) or whenever their is a new risk manager. For the 20220-21 school year, the deadline will be in the spring semester.

ITS Forms

  • TruView Announcement Request – If you are interested in sending out an announcement via TruView, please complete and submit this form to the Public Relations Office in MC 202. This form requires an advisor’s signature.
  • Student Organization Web Account Update/Request Form – This form is for if you are looking to change or set up a web account, or if you are wanting to allow new people to have access.  This form will be filled out and submitted to Web Services- ITS in McClain 111
  • Student Organization Email Account Request/Update Form – This form if to be completed if you need an email set up for an organization, need to reset the password, or need to update who has access to the email. This form will be filled out and submitted to Web Services- ITS in McClain 111

Physical Plant Forms

  • Tables and Chairs Request Form – This form is used to request tables, chairs, and trashcans from the Physical Plant.  This form should be submitted to the Physical Plant main Office in the General Services Building (GS) room 207.
  • Tent Request Form – This form is used to request the Truman State University Tent.  This form should be submitted to the Physical Plant main Office in the General Services Building (GS) room 207. Please contact the Physical Plant to learn more about the specific policies regarding usage of the Tent.
  • Stage Request Form – This form is used to request a stage for an outdoor event. This form should be submitted to the Physical Plant main Office in the General Services Building (GS) room 207. Please contact the Physical Plant to learn more about the specific policies regarding usage of the Stage.

  • Student Organization Constitution – Student Organization Constitution Guide. All of the things you need to know about your organization’s constitution. Things that are required are marked in red and things that are optional are marked in green.
  • Organization Roster Submission Guide – Student Organization Roster Submission Guide. All of the things you need to know about how to submit an updated roster for your organization.

Other Helpful Forms

  • Publications Work Order Request Form – Need a poster designed? Visit with our talent University Publication staff. This form should be completed and returned to the Publications Office in Barnett Hall 2220 in order to have work processed.
  • Printing Work Order Request Form – Need copies? The University Printing staff can help you with all your needs. This form should be completed and returned to the Printing Office in Barnett 1214 in order to have the order processed. Organizations that do not have a University Budget are required to pay upon pick up.
  • Travel Registration Form – This form should be used by all students traveling to and from an event/activity that are receiving financial support from Truman State University. This is especially important when that activity or event requires travel outside a 20 mile radius of campus.
